It sounds like the problem is with your PM or SDLC not including a risk / issue register and assigning responsibility to a person with a resolution date and appropriate follow through. Not really an issue for you as a BA but is sounds like no one else is doing it, so you may have to do it by default. Its basic / standard project management really.

I think risk management is very important for project management.
A risk management process can decrease problems on a project. 80 or 90 percent risk decrease by this process. In combination with solid project management practices having a well-defined scope, following a good change management process.
